Does the fly screen keep out midgies? If not, can it be removed easily in order to change the screen material so it protects against the dreaded midge

The screen can be taken off by turning the retaining clips half a turn each and then you could attach any material you have. Alternatively, add velcro strips and stick a midge net over the opening

Can i fit to high top van cabin

It depends on the profile of the roof your attaching it too, and if your installer is competent to create a flat sealable surface. The methodology for fitting this is almost identical to any 400x400mm roof light. An adapter plate to create a flat mount surface is recommended, but not essential. I'd recommend you check the overall dimensions of the unit against the roof space you have available and the shape. If your intending to use this unit while driving, it probably isn't suitable. It's designed to be open while static.
