Does pass the usb pass-through port block the usb port next to it if using on old mac book pro?

May do, plastic is quite thick could maybe shave it down or use a usb hub. I use a usb hub with switches so can easily switch off without unplugging.

Which way do the fans work? Do they suck down though or blow up and out?

I think the fans are reversible. I have set mine up to blow up through the base; my recollection is that I had a little difficulty determining which way up to mount them, on the underside of the aluminium base plate. I also wanted to be able to switch off the fans, but leave them plugged in, so I inserted a switch inline (see https://www.amazon.co.uk/gp/product/B07F7Q46CX/ ). Hope this helps.
