Do these run small ? I’m 5’10 and 159.3 lbs and I normally wear medium on shirts

Actually, I think they run very true to size. They are supposed to be tight fitting to accomplish the "compression". I got my husband the XL. He is 6 feet tall and weighs about 230. The medium should be good for you.

Will this make me look okay with 42 inch chest/34 inch waist? or will the small love handles show poorly?

Should not if you are wearing right size pants. Pants should be 1” larger than waste size for proper shirt fit.

How many shirts are in a package?

Only 1 and I did not realize that they were compression

Does this help tone muscles and get rid of belly fat over time?

No, it won't. The only effective way to improve muscle tone and body fat is the right combination of diet and exercise. What this shirt does do for me is provide a little support to my muscles when engaged in movement because of the snug compression feature of the design and material.
