How large is this hat? Does it sit over the entire head or is it something that you can set on your head at an angle?

It is not that large. I used it for my husband's funeral. I did set it at an angle. Cute hat. Bad occasion. I will never probably wear it again. It's like a keepsake from the funeral.

Is the top of the crown flat? Is it lined?

Yes, the top is flat. It is not lined... And to be honest the quality is so-so. ( a lot of hot glue) it worked for me as part of a costume, but don't recommend it as something that you plan on wearing for more than that purpose.

Does it stay on your head without having to pin or clip it in?

I bought this over a year ago. It’s beautiful, but I would say yes to having to secure it to really stay on.
