Does the hat literally just say "embroidered hats your text here?"

No ,you type your custom words. Be careful though mine took up two lines and the manufacturer put them on backwards. I am still waiting for a replacement.

How many lines of text do you get, 2 or 3? Hat in ad seems to imply 3 but I don't know if they're counting the very top line.

Good, heavy hat. Best lettering selection

Hats are of good quality, heavy cloth (not for keeping cool.). Stitching is very good. Only company I found which had a larger font for top row and allowed adequate letters on bottom row (almost) Detectives who have seen them in my office have asked for them and so I will have to re-order without the time crunch.
