Do you have to buy a cover for the heater? I am worried about my koi getting burnt....

No. I have 12 large fish, and I've had these heaters for 12 years without ever having a problem. The guard is to prevent the unit from sitting directly on the liner and melting it "over time", and to allow the heat to come out the bottom of the coil too, but it never gets that hot to harm a swimming fish. My fish can be found congregating above it for the extra warmth all winter.

I have a 4'x8'x3' pond and cannot find a good pond heater/de-icer. Question, what is the estimated water temp when the air temp is below freezing?
