Hello, Yes our Elance style is a 100% cotton underwear, with a 100% cotton gusset.

Is the crotch black on the inside?

I only wear white but if the underwear is black the crouch is probably black. That is why I wear white only.

A size medium equal is equal to what size, a 6 or 7?

Not sure exactly what a medium is. I wear size 14 Jean and prefer a size 7 panties. I have both 6 and 7. Not a huge difference between them. Great panties.
