You need to buy the whole kit. You can call Toto directly. But I’m about 99% sure that comes all together.

I don’t know. Ours is 2-piece toilet with the top-of-tank flush.

Q: Does it come with the template?

Sorry for the late response. This is the toilet mounting kit THU742#01-A and no it does not come with a template.

Q: I don’t see holes for the side screws to go into. Where do the side screws attach?

There is no side screw holes. The manufacturer has done this intentionally. It creates alinement flexibility when placing the toilet. Once you have placed the toilet, you can pre-drill a small pilot hole through the blocks. This will allow for easier side screw installation.
