Can this machine be set up on a carpeted floor?

Sure, no problem. The machine does also come with a 2' x 3' mat which you can either put under the machine if you want to protect your floor or carpet, or in front of the machine if you want to have some padding for a part of your body that is off the machine when you are exercising - for example for you knees or feet if you are doing a push-up exercise with only your hands on the machine. And if you have a carpet you can use the carpet for this padding for your body and the mat under the machine to protect your carpet from any marks from the feet of the machine.

Who is the manufacturer of this product, are they in the usa. how long has this unit been made and can parts be purchased after the warranty expires ?

In order to keep the price affordable for more people this product is not produced in the US. Comparable US made vibration machines cost two to three thousand. After warranty expires repairs are available through Vibrant Health (bcvibranthealth.com). Please save the original box and packing materials for shipping.

Is this the same machine as a VT003F?

No, this machine the VHPower 1000 is not the same machine as the VT003F, nor made by the same manufacturer; however they are both liner or vertical vibration as opposed to pivotal oscillation.
