I have just taken delivery of 2 and the are saying very different temperatures both of which appear inaccurate for a fridge. What’s wrong .

Poor quality. As I said in my review the 2 I had showed different readings in the same place. One “appeared“ to be right but I have no way of checking. Send them back.

Can this thermometer be kept in the fridge and freezer for an unlimited amount of time or is damage likely to occur if left for to long?

Mine still work fine after a few months, I assumed they would be designed to be left in as a permanent feature and so far so good.

Is there any way to reverse the hook? The way it is now, the display faces the wrong way when you hang it on a freezer drawer; pretty dumb design.

Yes it's easily reversed. Gently unclip one side of the hook hinge, flip it over and click into place. See my review with photos.

Is this only for use for fridges?

Within a freezer I have found its minimum registered temperature is -19.5°C. It would go down to this level outdoors. I recently defrosted my larder fridge, leaving the thermometer on the kitchen counter. It registered the room temperature at 22.2°C. I do not know its maximum, but yes it can be used outside a fridge or freezer.
