Blown the T12 bulbs immediately. Does not come as a two pack as description says!!!!
The description says that this is a 2-pack of ballasts. I received it with only 1 in the package. The ballast doesn't work anymore and the ballast also blew the two T12 bulbs I installed. Following instructions: white wire to live, black wire to neutral, red wires for one bulb, blue wires for the other bulb and a ground wire connected to the light fixture. Both bulbs tried to light, but didn't fully turn on and just didn't work after the initial flash of light on one side. 120vac at 60hz outlet.

It Fixed the Problem
The ballast fixed the problem after I determined that the bulbs were fine. I needed an experienced electrician to install it, but I expected this. The charts were very helpful in determining which ballast to order.
