I love this kid friendly pain rating card. It's easy to be understood.

Very useful for all ages
As a reference tool, I use this even with adults of all age ranges who seem to have difficulty giving their pain a number. Great for kids. Some hospitals put the tools on a wall, but I find it easier to carry with me and have at the bedside or during triage.

Save the ink!!!
The ink will rub off after a while so I suggest putting a coat of clear nail polish on before use.
