Q: What size would one wear with waist around 40"? need to order soon and not happy with ones he has used they fall apart. m/l or xl?

I was say XL. WALMART Assurance work well too. Don’t buy the Walgreen’s brand. Pure junk.

Q: is there any way to keep them from falling off your butt?

I have not had that problem with TENA, as I DID with Depends, which never came to the waist. TENA is a great fit for me.
