Q: Is the cap/cover a "screw-on" type, or simply "snug-on (not screwed on"? When the cap is simply snug-on, the cap leaks (big problem!)

it's screw-on(the cover which between long hose and urinal),but the area of other cap in the adapter is snug-on.we don't know where do you point.

Q: Has the mfr read the less than 5 star reviews and made changes to to improve the product?

I have not seen any changes. Although the unit is solid, easy to use and somewhat built solid. My only change would be the volume. If the user will be alone a considerable time and uses the unit twice it’s my experience it’s full. But if it will be emptied as used this unit is suitable for most patiences.

Q: Do you have a larger than 5.5 cm urinal that could be switched onto this unit. We bought this unit but a larger urinal end would be helpful

Q: I sit the bottle on the floor at night and there are times when the urine backs up the tube and gets on the bed even though it’s less than half full.

You can cut hose as Shown in photograph to length needed to your needs.
