Q: How can you tell which size to order?

There's a sizing guide on this page of the website: http://www.woodyknows.com/Products.aspThey used to sell cheaper frames (without filters) for sizing purposes, but I don't see that on Amazon anymore. Based on my nose measurements, I initially ordered the IIIR, but I later figured out the IIIS fits better. My "nares" are round, but the interior of my nose is narrower. In short, the sizing is tricky because all noses are different. But it's worth the effort. The filter has really helped me. Best of luck.

Q: does this item help reduce second hand smoke?

Please be kindly noticed that both of WoodyKnows nasal filters is expected to filter particles, such as, pollen, dust, dust mites, mold spores, pet hair & dander, but can NOT filter odors, fragrance or this kind of chemicals. We are developing our odor eliminating filters which will be launched in 2-3 months and it mainly works for harmful gases, smoke, vehicle exhaust. For organic fragrance, e.g. fragrance of cosmetics, it works well but not for inorganic fragrance. Please stay tuned.

Q: Can i buy filters without the frames?

Yes you can. You can find them on amazon just read the disruption or title. It will say "replacement filters." They have the allergen and carbon filter ones.
