I don't understand the question. The receptacle is just that, no instructions, no extra optional pieces, just a 50 AMP 250 V receptacle that matches the plug on some welders with up to a 50 AMP draw. The feeling I get from Leviton is If you don't know what your doing don't buy our product.

Q: Can I use a 3 prong and 4 prong wired appliance on this outlet? I have a welder that has a standard 3 prong 220v plug and I need an oven outlet.

This receptacle is designated NEMA 14-50R. It will only mate with NEMA 14-50P plug. If you are planning to plug other than the latter, it is not going to work. You need to make an adapter (NEMA 14-50P plug + cord + same type receptacle as your plug) so that you can use this receptacle. You should also be knowledgeable as to wiring so that there are no miswiring or electrical code deficiencies. If you are unsure, ask a qualified electrical pro.

This is an "RV" 50amp-120/240 receptacle, it is also the same as a "Stove plug" with some slight difference. So to answer your question: Yes one side of the receptacle has a larger slot then the other.

There's formal, like for a permanent 50 amp installation that has to meet code, and then there's practical. If your car is e.g. a Tesla Model 3, it won't pull more than 32 amps no matter what, and you can throttle it down, say to a bit under 30. Then look at what's used in 30 amp extension cables, and go from there.
