Q: What is the difference between this and the MKR-26?

If remember correctly the mkr 28 comes with everything to make your connections and it was a thicker gauge wire. I decided to pay the extra $25 for thicker wire and all the connections. This is the best trolling motor plug I've ever used. I figured since I paid $3500 for my ultrex, $150 is cheap insurance to never worry about that plug ever again

Q: Is this water tight enough to be able to mount it on a flat surface where the receptacle is basically open up to the sky?

Yes, that is how mine is mounted. Switching to this connection is a must for anyone who is a serious user of a trolling motor. Installation isn't terribly difficult except for the fact that the hole for this connection is likely larger than your current hole. Make sure you use a hole cutter that has both the existing size and the new size connected.
