Q: I'm installing this in a 2x4 wall with a 2x4 panel. What is the the clearance needed for it to open and close smoothly?

We have about 1/4 inch clearance. However I would check further, these hinges do not return the doors to middle automatically.

Q: Can you use this hinge for a hidden bookshelf door? Outswing and mounted from the left corner? Or does it have to be a center mount?

I used the hinge on a paneled wall on a hidden door, I set it in from the edge 3 inches, the panel opens out and worked great.
