Is the inside waistband cloth covered?

For undershorts with a waistband that is covered on inside (to protect skin from contact with elastic / latex material) try Lands End.

Have always worn Large 38-40 in these. Just saw new sizing Large is 36-38 and XL is 40-42. Which way should I go on this?

Depends on what your waist size is. If you are closer to 36 or 42, go with that particular size.

What do they mean by 3" high, 6" wide, etc.?

I didn't notice that. I would imagine the 3" is an inseam. 6" might be width of leg hole but I have no idea? My husband has muscular thighs and these fit well.
