Q: Will this lie fully open and flat? It doesn't look like it will.

Not open but unopened yes. I had gotten it for my son to use as is diabetic kit. It works great. Holds everything. Even keeps his insulin good in the cool part. Doesn't need to be refrigerated after opening.

Q: will this unit accept a small bottle of orange juice?

Yes, this can hold a small orange juice. To judge the interior space better, I can stand up two 12 oz cans of pop side by side and lay a 3rd sideways over them to completely fill the space.
