How is the medallion ID adjustable up to 6.5"?

The actual hole is 4-1/2 inches ID, but there is a flat area around that opening and that is what allows the canopy to be as large as 6-1/2 inches. You can either cut the opening larger or allow the canopy to fit on top of the flat area.
