Will this help restore a glove that is flat?

Wrap will help maintain shape of a glove that is well taken care of. If "flat" means soft or floppy, then this will not help that.

does this item help break in a new glove?

It helps to keep the form of the ball when not in use. Break in, I believe, comes from use of the glove. This just prevents it from getting that nasty flat fold.

Should I store the glove wrapped? Or just wrap during the break-in period?

I leave mine wrapped. Not as tight as I would for break-in but wrapped (with ball in pocket)

Is the included wiggle ball an 11" or 12"?

I believe 12". Mine's currently being shipped. I use it in my Adult Glove, and use a regular Baseball with my Son's. Seems to be building a pretty solid pocket in both gloves. Hope this helps.
