Q: It is suddenly stop working, horizontal lines & no image. What's wrong?

Check if there is a break in the signal wire near the USB (White wire) you may have bent it too far. It only takes a fraction to do harm to such a tiny wire.

Q: How do you get an image to appear on the screen with a chromebook? Plugging in the usb does nothing.

It seems that OS for a chromebook is "Chromium" which is not fit for the Celestron Digital Microscope imaging. The instrument (Celestron DMI) works on Windows PC.

Q: I know this camera will not resolve at 5mp using photo booth, however, will it resolve at 5mp using windows 10 camera app?

Nope. 2.1 mp is what I get in Windows Camera producing 16.9(1920x1080) images.
