What are the center of the screws for the face plate?

The two screws are what hold the horn in place. The horn goes on the inside of the boat. You need to have a large hole in the bulkhead for for the horn sound to come through and a small screw clearance hole on either side of that. The two small holes are where the screws go through to hold the face plate on the horn with the bulkhead sandwiched in between. It's a great horn. I'm very happy with mine. I hope this helps.

do the mounting screws come with the unit?

Pretty sure the screws came with it. Been awhile since I installed it.

Mine was purchased almost 10 years ago. It had a membrane over the horn opening, but I can't remember if it seemed waterproof but that membrane didn't last long. The bottom of the horn has a drain and short tube, so any water should be drained out before getting into the electric components. Mine is working file without the membrane and with the drain feature.
