Is the Hubbell 5324-0 box metal or plastic?

It's made of metal -- my guess is aluminum.

How does it connect to the wall, in my case stucco, over an existing gang box.

2 mounting lugs and self-tapping screws are included for fastening the box to the finished surface. If you desire to mount over an existing box mounted inside the wall, we recommend using a weatherproof box extension and they can be purchased with or without hubs in the event you are installing conduit.

Are the holes blocked or open like the photo? How weather proof can it possibly be with a giant hole in the top?

The threaded hubs are open. The closure plugs are included with the box, and must be installed using proper sealing compound (RTV silicone) into any unused opening.
