Can another piece of paper be added to the stack without removing the clip once clipped or does it have to be removed & reclipped?

No clue. The Kinokuniya Japanese stationery store here in Portland carries these but I've never looked at all of the sizes they have.

How do I load the refills

You push them into the top opening as you use them. I love these. They don’t fall and not bulky I keep one in my computer bag and in my desk. Not a fan of paper clips.

Is the tiny silver strip across the top of the dispenser supposed to be removed prior to use?

I would say yes. When I bought mine it did not have a strip at all. So I would think it should be removed
