Temperatures appear to be thermostatically controlled at levels determined by the control setting. The heating element shuts off when the desired temperature is reached and turns back on when it falls some predetermined amount. The frequency of power cycling would be determined by the thermal characteristics of the object being heated and the ambient temperature.

Will this adjust automatically to 220 volts?

I am afraid the Electric Mini Stove can not be adjusted automatically to 220 volts,it is 110v.

Does the bottom of this burner get hot? I am planing to use it on my wold desk to heat up teapot, not sure if it will burn my desk.

No, not hot enough to burn your desk, but this is hardly the best choice for your application. You will be placing a pot of boiling water on a 3 7/8" round disk elevated 3" above the desktop. Sound safe? For another $10 you can solve that issue with a Topwit Electric Hot Pot Mini. Safe, dual wattage, stable and fast. The unit's footprints are nearly the same and electrical requirements at max temp vary by only 100W. I use both, 2 hot plates for cooking in a designated area and a hot pot for fast coffee and tea (or whatever) prep at my desk or kitchen table. They're both great units, but safety first around boiling hot water. Good luck.

No, not directly on the plate,you need use a mini pan that less 4” in diameter.
