Q: Is this just pure lard without bht? and does it smell like good bacon or is it rancid or smells overpowering like their mutton tallow?

Since it has the USDA seal, I'm willing to take a chance and say no. As to the smell before heated, there is none. After cooking with, it offers up the smell of the product being cooked. Pork lard is so good that I pay the additional cost above ordinary cooking oils.

Q: Are these grass fed and grass finished pigs?

I hope not! Pigs don't eat grass naturally. Corn is the preferred pig feed. Why would you want a pig to eat grass? Would you feed a cat grass? Same thing. To ask that pig be fed grass and not their natural diet of corn is like asking for a cow that is fed corn and not grass!

Q: Does this need refrigeration after opening?

Generally, I don't, but, they recommend that you do refrigerate.
