Q: Do they mount photo frames flush to the wall, i have some gold hooks but they push top out,

So the T-bolt fitting itself is flush with the wall. The hook that you use to attach things to the wall is held by the screw that is part of the T-bolt fitting. This hook is just like any other picture hook and so yes, if you hook a cord attached to a picture over the hook then the picture will stick out a bit at the top. The trick would be to use a metal fitting screwed directly into the picture frame or, tie the picture cord really tight. The advantage of the T-bolt is that it will support very heavy weights on just plasterboard, which is not a strong material, so the T-bolt might be a bit over the top for just a picture. I used mine to attach a large heavy mirror with a shelf to a plasterboard wall. I estimate that it weighs around 15 kg. It hasn't fallen off yet!

Q: Could this be used to secure coat hook / rack?

I used these to secure a metal coat rack and it's holding loads of heavy coats with zero movement. I'm seriously impressed. If your'e hanging a coat rack with a wooden backplate then you'll need longer 50mm countersunk M5 screws to be long enough to go through the wood and and the plasterboard. Good luck.
